I think we do not need the word Form on any of the forms. It is redundant. To apply you must fill out a form.

I had some changes to the cover letter that I forgot to send yesterday. I like what Estelle suggested as a name for the Youth Advisor.

Catherine, my thanks as well for the work you've done to create these forms.    Since the YP calls the person a Conference Youth Advisor, perhaps the form should be :  Youth Advisor Application Form. Program could mean that we are conducting the program, and may not imply that anyone working with youth in any program has to file this application.

Catherine, Thanks for all your work. I agree that the parents should print their names.

I would use the term: "Application to Work with Youth" . The applicant is identified only at the end. I assume it will be attached to the Volunteer Infornation form or do we need to ask for more info? Dan

All.............I don't see a reason to change it, but if we're going to, let's make it more to the point.

How about : " Application for Youth project TC Guides " or "Application for TC Youth Project Guides" ????

Jane,

I agree the word "form" is unnecessary, and I like your suggestion for the Youth Worker application. The policy manual refers to it as the "Youth-Program Application Form for Employees and Adult Volunteers" which I felt was too long.

Do the other committee members agree with removing the word "Form" from the forms' titles; and changing the Youth Worker form title to "Application to Work with Youth" or " Working with Youth Application", or similar?

A couple of changes -

Do we want to include the word Form in the titles?  To me it seems unnecessary

If I saw the title Youth Program Application, I would think it was for a youth to apply. WouldWorking with Youth/Minors Application be a better title?

Catherine................looks good and I thank you for your work on these forms.....if I may add a small comment. On the Parental Consent Form, I think the bottom section where signatures are required.......should be doubled spaced, because from past experience, they tend to scribble the names. Confining them to a small space tends to excerbates the situation. I would also request them to "Please Print legibly."

 

 Also, on the lines requesting the Parents and the Physicians phone numbers, I would also ask for the Cell phone Numbers. Often times when the child is occupied with a project the parents take-off to go shopping or wherever, and they're not at the primary number.    

 

On the Youth Program application Form........ I would increase the size of the type face on the bottom "Confidential statement', so there is no question as to the secure archiving of this sensitive information.

Hello everyone,

I have modified the new Volunteer forms based on our previous meeting's discussions. There are 3 forms attached in one file "Volunteer Forms.doc":

(1)   Volunteer Information Form - all volunteers will be required to complete & sign this form;

(2)   Parental Consent Form - Parents or Guardians of all minor volunteers must sign this form; and

(3)   Youth Program Application Form - to be completed & signed each year by all volunteers who wish to work with minors.

I have also attached a copy of the latest draft of cover letter for the Youth Policy. Please mark up or let me know of all changes and suggestions you have regarding the above documents.

 

I haven't finished the Incident Report form yet; nor have I looked over the latest draft of the Youth Policy; but I will be cross-checking the Policy against the new forms to ensure that they agree.

 

At our last meeting, I mentioned that the Volunteer Center of Bergen County is holding a Volunteer Celebration for any volunteers we choose to recognize.  I have since found out that unfortunately, there is a $10 charge for each volunteer, staff or guest who will be attending the event.  So we need to decide if we want to use some of our Volunteer Recognition funds for this and if so, how much.  The registration deadline is April 1, 2010.
